To minimize the time it takes from your first session, all our intake paperwork will be provided to you in advance of your initial intake session. The paperwork is completed directly on our secure Client Portal to ensure privacy. Once you have completed the contact form we will be in touch with you by phone to answer any questions and schedule a time for your initial appointment. You may also contact us by phone to begin your intake process at 630-377-5105.
Once you’ve contacted us by phone or email, we’ll email you the link to your Client Portal account to complete the initial intake forms. After you receive the email, simply click on the link contained within to fill them out. All forms are located on an encrypted server in order to maintain compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and eliminates the need to transmit the information via email. We maintain complete confidentiality for all our clients and take extra measures to ensure their privacy.
